Imperia Counseling is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team, providing essential support and services through a unique remote work environment specifically designed for hospice care. This position is unique as it operates from a remote setting while engaging directly within a hospice center framework. Our innovative approach allows our Licensed Clinical Social Workers to perform evaluations, assessments, and provide emotional support from a location that complements both patient needs and employee comfort. Key Responsibilities

As a Licensed Clinical Social Worker at Imperia Counseling, you will:

  • Conduct comprehensive evaluations and assessments of patients' emotional and psychological needs.
  • Participate in creating and implementing Individualized Family Service Plans (IFSP) tailored to each child’s unique circumstances.
  • Monitor and assess outcomes as a collaborative team member to ensure the effectiveness of interventions.
  • Provide critical support and resources to families, equipping them with the knowledge and skills needed to foster their child's development.
  • Develop rapport with children, families, and team members to promote a nurturing and effective learning environment, using evidence-based practices.
  • Facilitate joint planning with families, recommending strategies that adhere to the Natural Learning Environment Practices.
  • Employ the coaching model, integrating natural environment teaching techniques to build developmental skills.
  • Ensure compliance with federal and state regulations while promoting a safe health environment.
Patient Population / Client Focus

Your role will predominantly focus on supporting young children, including infants and toddlers diagnosed with disabilities or developmental delays. You will play a critical part in their journey, directly influencing their growth in vital areas such as physical, cognitive, social/emotional, and communication skill development. Engaging with families, you will guide them through difficult times, helping them access necessary resources and interventions to support their children’s unique growth trajectories.

Qualifications

The ideal candidate will possess:

  • A Master’s Degree or Doctorate in Social Work.
  • Active licensure as an LCSW through the Department of Public Health.
  • A valid driver’s license to facilitate travel as necessary.
  • A strong commitment to adapting therapeutic techniques to meet diverse learning styles and individualized patient treatment plans.
