Key Responsibilities
  • Conduct comprehensive evaluations and assessments of children and families to develop tailored support plans.
  • Actively participate in the creation and implementation of Individualized Family Service Plans (IFSPs), ensuring each plan is aligned with the child’s unique needs.
  • Collaborate with a multidisciplinary team to monitor and evaluate progress, adapting strategies as needed.
  • Engage with families through direct interactions, providing education and resources that empower them in their child’s development.
  • Ensure compliance with all applicable federal and state regulations concerning early childhood intervention.
  • Build trusting relationships with children and families in order to create a supportive and effective learning environment.
  • Implement evidence-based practices that align with Natural Learning Environment Practices to promote child development in everyday settings.
  • Utilize a coaching approach to educate families in natural contexts, enhancing the learning experience for children.
  • Maintain meticulous documentation of all interactions, assessments, and plans in accordance with legal and organizational guidelines.
  • Proactively safeguard the health and safety of both clients and team members in all settings.
Patient Population / Client Focus

Your primary focus will be providing support for infants and toddlers with disabilities or developmental delays. You will play a crucial role in helping these children acquire essential skills across multiple domains, including cognitive, physical, social/emotional, and communication abilities. Your work will have a direct impact on their families, promoting both individual growth and overall family dynamics.

Qualifications
  • Master's Degree or Doctorate in Social Work from an accredited institution.
  • Active licensure from the Department of Public Health; LCSW is required.
  • Possession of a valid driver’s license and reliable transportation for site visits.
