This remote position allows for flexibility while maintaining close interaction with an interdisciplinary team of health professionals supporting our pediatric hospice center. Although you will primarily work from home, regular virtual meetings will facilitate collaboration and communication with team members. The role also requires periodic visits to local facilities for assessments and family support, ensuring a comprehensive approach to patient care.

As a Licensed Clinical Social Worker at Synergy Counseling, your responsibilities will primarily include:

• Conducting thorough evaluations and assessments of children enrolled in the hospice program to understand their needs and develop appropriate care strategies.
• Participating in the development and implementation of Individualized Family Service Plans (IFSP) focused on enhancing the well-being of each child.
• Monitoring outcomes and progress as a vital member of a multidisciplinary team, ensuring compliance with federal and state regulations.
• Providing emotional and practical support to families, helping them navigate the complex challenges associated with pediatric hospice care.
• Building strong rapport with children and their families to foster a supportive and engaging learning environment.
• Utilizing evidence-based practices and Natural Learning Environment practices to develop customized intervention strategies.
• Educating families on child development and providing resources to enhance their child's growth in both natural and clinical settings.
• Ensuring adherence to health and safety standards for all staff and individuals served.

This role specifically targets children with severe health challenges and their families. You will work with a diverse group of patients, including infants and toddlers experiencing disabilities or developmental delays. Your expertise in early intervention will make a significant difference in their quality of life during critical phases of development, particularly in a hospice context where families face unique emotional and logistical burdens.

The role requires:

• A Master's Degree or Doctorate in Social Work.
• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
• A valid driver’s license and the ability to travel to local assessment sites as needed.
• Commitment to comply with all federal and state regulations governing social work practices.
• Proficiency in adapting interventions to fit various learning styles and developmental needs.
• Strong organizational, communication, and interpersonal skills, with respect for diverse backgrounds.

Preferred candidates will possess:

• At least one year of experience working with children within early intervention settings, particularly in hospice or palliative care.
• A demonstrated commitment to Natural Learning Environment Practices to facilitate developmental growth.
• Bilingual abilities to engage effectively with a broader range of families.
