Synergy Counseling is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. This full-time, remote position is focused on supporting families and providing critical services to children facing serious health challenges within a hospice setting. Work Environment

This remote position allows for flexibility while maintaining close interaction with an interdisciplinary team of health professionals supporting our pediatric hospice center. Although you will primarily work from home, regular virtual meetings will facilitate collaboration and communication with team members. Key Responsibilities

As a Licensed Clinical Social Worker at Synergy Counseling, your responsibilities will primarily include:

  • Conducting thorough evaluations and assessments of children enrolled in the hospice program to understand their needs and develop appropriate care strategies.
  • Participating in the development and implementation of Individualized Family Service Plans (IFSP) focused on enhancing the well-being of each child.

  • Utilizing evidence-based practices and Natural Learning Environment practices to develop customized intervention strategies.
  • Educating families on child development and providing resources to enhance their child's growth in both natural and clinical settings.
  • Ensuring adherence to health and safety standards for all staff and individuals served.
Patient Population / Client Focus

This role specifically targets children with severe health challenges and their families. You will work with a diverse group of patients, including infants and toddlers experiencing disabilities or developmental delays. Qualifications
