Sentara Health Plans is hiring a Social Worker/ Masters in Social Work (MSW) for Maternity/OBGYN - Remote in Winchester, Shenandoah, Frederick, Warren, Clarke, and Page, VA!
Status: Full-time, permanent position (40 hours)
The position requires telephone and to conduct face-to-face home visits in the maternity member's home or other locations within Winchester, Shenandoah, Frederick, Warren, Clarke, and Page, VA!
Work hours: 8am to 5pm EST, M-F
Location: Remote in Winchester, Shenandoah, Frederick, Warren, Clarke, and Page, VA!
Job responsibilities:
Social Worker works in a collaborative role with all members of the health care team and community service agencies to coordinate care for Sentara Health Plan members to improve health outcomes. Assesses for barriers to compliance with the medical care plan and implements appropriate interventions to promote improved health and compliance with recommended therapy. Acts as a resource to assist members in need of community-based program assistance and empowers the member with the skills to navigate the process for obtaining appropriate medical care and community assistance.
Education:
  • MLD - Master's Level Degree Social Work REQUIRED

Certification/Licensure:
  • Licensed Clinical Social Worker (LCSW) preferred

Experience:
  • 3 years of experience as a Social Worker REQUIRED
  • Social Work in physician office setting, CSB, home health, long term care, acute care or managed care setting experience REQUIRED
  • Experience working with low and high risk pregnant population/Maternity/OB/L&D/Mother Baby/Postpartum experience preferred
