What is remote case management?

Remote case management is a process where case managers coordinate and oversee services for clients, such as patients or social service recipients, from a remote location using digital communication tools. This approach allows professionals to assess needs, develop care plans, monitor progress, and provide support without in-person meetings. Remote case management is increasingly popular in healthcare, social work, and insurance sectors, offering flexibility and expanded access to services. It relies on secure technology to maintain client confidentiality and ensure effective communication. This model can improve efficiency and client engagement, especially for individuals in rural or underserved areas.

How does remote case management typically facilitate effective collaboration with interdisciplinary teams?

In remote case management, collaboration with interdisciplinary teams is often achieved through regular virtual meetings, secure messaging platforms, and shared documentation systems. Case managers coordinate care by maintaining clear communication with healthcare providers, social workers, and external agencies, ensuring that all parties are updated on client progress and needs. While working remotely can present challenges such as time zone differences and technology barriers, most organizations provide access to digital tools and scheduled check-ins to support seamless teamwork. This collaborative environment enables case managers to deliver comprehensive and coordinated care, even when working from different locations.

What are the key skills and qualifications needed to thrive as a remote case manager, and why are they important?

To thrive as a Remote Case Manager, you need a background in social work or related fields, strong organizational skills, and experience with case management processes. Familiarity with case management software, electronic documentation systems, and sometimes certifications like CCM (Certified Case Manager) are typically required. Exceptional communication, empathy, and problem-solving skills help build rapport and effectively support clients remotely. These skills ensure efficient service delivery, client engagement, and successful outcomes while working from a distance.

Our organization prioritizes person-centered, holistic care while utilizing evidence-based advancements in neuroscience to facilitate the therapeutic process. We believe that transformation begins with quality support and collaboration. Our team is built on mutual respect, shared values, and a commitment to uplift all individuals who seek our help.

Work Environment

This position offers the flexibility of remote work, allowing our mental health professionals to create a balanced work-life schedule while managing outpatient therapy sessions. While our therapists primarily operate from home, they have access to a strong intra-office support network and participate in regular team meetings to discuss case management and ongoing training. Being part of a remote team does not mean you will feel isolated; rather, you will work within a culture that promotes connection, professional growth, and open communication.

Key Responsibilities

As a Licensed Mental Health Counselor, you will:

  • Conduct culturally sensitive, strength-based assessments for new patients and their families to gauge their therapeutic needs.
  • Provide individualized therapy sessions for patients, as well as family counseling, tailored to the specific needs identified during assessments.
  • Complete thorough billing documentation, individualized treatment plans, and necessary insurance authorization forms, ensuring compliance with industry regulations and timely submissions.
  • Develop comprehensive crisis plans, safety plans, relapse prevention strategies, and wellness plans to support clients in maintaining stable functioning.
  • Create effective discharge plans to ensure continued care and support post-therapy.
  • Adhere strictly to all federal and state laws, regulatory requirements, and NEHS policies to maintain the organization's high standards of care.
  • Engage in ongoing professional development through training and education to remain competent in the latest therapeutic practices.
  • Participate in case consultation meetings and agency gatherings to share insights, discuss challenges, and foster teamwork.
  • Complete all required licensure and training to maintain your professional credentials.
Patient Population / Client Focus

Our outpatient center serves a diverse population of individuals and families seeking mental health support. We focus on delivering personalized care that caters to varying diagnoses such as anxiety, depression, trauma, and relational issues. Our clients range from children and adolescents to adults, reflecting the wide array of needs within our community. Our goal is to create an inclusive environment where all patients feel safe, respected, and empowered to embark on their therapeutic journeys.

Qualifications

To be considered for this position, you must possess:

  • A Masterโ€™s degree in social work, counseling, marriage and family therapy, or a related field that qualifies you for licensure.
  • Current licensure in the state of Georgia as a Licensed Mental Health Therapist, such as LPC, LMFT, or LCSW.
Preferred Experience

While not mandatory, the following experiences are advantageous:

  • Previous experience working in an outpatient setting and utilizing evidence-based methodologies.
  • Familiarity with telehealth platforms and remote therapy techniques.
  • Experience in collaborating with multidisciplinary teams to provide comprehensive care.
Compensation and Benefits

We value our team and offer a competitive annual salary ranging from $80,000 to $100,000, commensurate with your licensure and professional experience. Additional benefits include:

  • Generous time off policies to promote a healthy work/life balance.
  • Comprehensive health and dental insurance packages.
  • Flexible schedules and the option for both full-time and part-time roles.
  • Short- and long-term disability insurance to protect your wellbeing.
  • Life insurance plans for added peace of mind.
Career Growth Opportunities

At Evolve and Change Mental and Wellness Center, we prioritize professional development. Our commitment to employee growth includes access to continuing education opportunities, workshops, and training sessions that enhance clinical skills. We encourage team members to explore leadership roles within the organization, paving the way for advancement in your mental health counseling career.
