Wellmore Health Center is seeking a dedicated Full-Time Licensed Clinical Social Worker (LCSW) to join our innovative remote Detox Center team. Work Environment

This full-time LCSW position is primarily remote, allowing you to provide vital services to clients from the comfort of your home while still being a vital integral part of our supportive team culture. You will benefit from a flexible schedule, with core work hours designated as Monday through Friday, from 9:00 AM to 5:00 PM. Engagement with collaborative tools for communication and case management will facilitate effective teamwork regardless of physical location.

Key Responsibilities
  • Act as a key liaison between clients, families, and outpatient service providers, ensuring clear and effective communication throughout the care continuum.
  • Regularly interact with insurance companies to facilitate pre-certification and manage utilization effectively, ensuring that client treatment plans are aligned with insurance requirements.
  • Participate in discharge and aftercare planning processes, ensuring seamless transitions for clients returning to their communities.
  • Provide therapeutic meal support while modeling healthy relationships with food and nutrition, which is essential in supporting recovery.
  • Deliver safe, empathetic, and affirming care tailored to the unique identities of each client, recognizing and integrating their diverse backgrounds in treatment.
Patient Population / Client Focus

The LCSW will engage primarily with clients in a detox setting, which includes individuals dealing with substance abuse and mental health challenges.
