Key Responsibilities

  • Engage in home visits as required for observational assessments, maintaining accurate documentation of findings to inform treatment strategies.
  • Facilitate the implementation of program plans by ensuring that therapeutic approaches are carried out effectively and meet the individual needs of each client.
  • Participate in interdisciplinary team meetings to collaborate on cases, share insights, and continue professional development.
  • Document client progress through detailed monthly and quarterly reports, ensuring all records are updated and available for team review and compliance.
  • Organize and lead training sessions to ensure all staff are proficient in program implementation and relevant documentation, fostering a culture of continuous learning.
  • Provide ongoing follow-up through interviews, home visits, and observations to guarantee lasting support and identify any necessary adjustments to treatment plans.
  • Act as a liaison with the broader mental health community, establishing connections that can enhance client care and program resources.
Patient Population / Client Focus

Qualifications
