The Community Health Worker contributes to the administration of care management. You will provide non-clinical support to the assessment and evaluation of member needs and requirements. This support aims to achieve or maintain optimal wellness. You will guide member/families toward and facilitates interaction with appropriate resources for their care and wellbeing. Community Health Worker assignments are typically straightforward and of moderate complexity while working in a hybrid environment with IN PERSON visits being required.

Job Description

The Community Health Worker builds visibility and credibility of Humana's products and services throughout the community with chats, posts, and interactions. You will engage new community members and strategize methods to grow communities and lead connections among all community members. The Community Health Worker understands their own work area professional concepts and standards, regulations, strategies, and operating standards. This individual makes decisions about their own work approach and priorities and follows direction. Managers manage and often guide work according to precedent and/or documented procedures/regulations/professional standards with some interpretation.

  • Conduct in-person and telephonic non-clinical assessments to understand member care needs, preferences, and socioeconomic barriers, and evaluate the home environment
  • Work collaboratively with other Humana associates as a member of Humana's Care Management Support teams, including care managers and Wellness and Information Navigators
  • Be a member of enrollee multi-disciplinary care teams (MDT)
  • Assist member with scheduling physical health and behavioral health office visits and addressing barriers to appointment attendance
  • Assess enrollee barriers to healthy living and accessing healthcare services and arrange necessary social services and supports
  • Be a enrollee advocate with providers, community resources, schools, and others, including accompanying member to provider office visits
  • Support enrollee compliance with their care plan, medication, and treatment adherence
  • Support enrollee self-management through the provision of culturally appropriate health education and coaching
  • Serve as an important knowledge source for community services and information for member and other Humana associates
  • Attend Humana community events to connect with member and provide education on case management services
  • Develop relationships with providers and community resources to support enrollee identification and referrals
  • Implement community assessments to identify community resource gaps

Required Qualifications
  • High School Diploma or equivalent
  • Minimum two (2) years of experience with community resources, health agencies, or social services (e.g., Area Agency on Aging, DME providers, Meals on Wheels)
  • Must reside in Oklahoma and work full time and travel to member's location
  • Intermediate proficiency in Microsoft Office
  • Respect for cultural and demographic diversity
  • Strong written communication skills and advocacy for members at all care levels
Preferred Qualifications
  • Community Health Worker training and/or certificate or willingness to complete within one (1) year
  • Bachelor's degree in social work or related field
  • Familiarity with state Medicaid program guidelines
  • Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy
  • Fluent in Spanish, French Creole, or Vietnamese

Workstyle: Combination remote work at home and onsite/home member visits

Mileage reimbursement is provided for work-related travel. Eligible mileage includes:

  • Travel from your home to your first work location of the day.
  • Travel between client or assignment locations during the workday.
  • Travel from your final work location back to your home.
Work at Home Requirements: To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
