About IMPaCT Care

At IMPaCT Care, we exist to remake the American health workforce from the ground up. We find, train, and support Community Health Workers people who keep their communities healthy through relationships, not just referrals. Our model is the largest, most scientifically validated CHW platform in the country, active across 22 states and trusted by over 70 organizations including Kaiser Permanente.

About the Role

This is a full-time position with IMPaCT Care. You will be a direct employee of IMPaCT Care, working embedded within our Kaiser Permanente partnership in your region. As a Community Health Worker, you'll be a vital link between high-risk patients and the care and services they need working in the community, not behind a desk.

What You'll Do

  • Build trust through consistent presence and culturally aligned, empathetic communication
  • Identify and address barriers to care: food insecurity, transportation, housing, and more
  • Connect patients to care teams and community resources; follow up to ensure needs are met
  • Document all client interactions, needs, assessments, and outcomes in the EPIC EMR system
  • Complete SDOH screenings and update care plans in a timely manner
  • Participate in integrated care huddles and collaborate with clinical teams
  • Use performance data and feedback to actively refine your approach and improve client outcomes

What You Bring

Required

  • High school diploma or equivalent
  • 13 years in community-based outreach, health education, or related work
  • Valid driver's license, personal vehicle, and car insurance
  • Ability to pass background check and pre-employment screening
  • Flexibility to cover multiple locations within your assigned region

Preferred

  • Experience with an EMR system such as Epic
  • Familiarity with Medicaid, D-SNP, waiver programs, and recertification cycles
  • Experience with SDOH screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) or case management platforms
  • Knowledge of HEDIS metrics or risk stratification concepts
  • Strong ties to community organizations: faith institutions, shelters, food banks, or mutual aid networks

Your Day-to-Day

This is a field-based position. You will spend the majority of your time in the community in clients' homes, in hospitals, in clinics, and at community organizations. You will report regularly to Kaiser Permanente locations for team meetings, training, and care huddles.

A typical week may include client home and community visits, care huddles with the clinical team, documentation and care plan updates, and phone and text outreach between visits.

Schedule: Monday through Friday, 9am5pm. Occasional flexibility may be needed to meet client needs.

Pre-Employment Requirements

All offers are contingent upon successful completion of:

  • Background check and pre-employment drug screen
  • Health and medical clearance, including proof of required vaccinations from a primary care MD

Required vaccinations/health screenings: TB screening, Hepatitis B, Measles, Mumps, Rubella, Varicella, Tdap/Pertussis, Influenza (required August 1April 30)
