The Guided Behavioral Reflection Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position based in Aurora, Colorado. This role requires expertise in addressing the psychosocial needs of seniors, collaborating closely with clients and their families to promote emotional well-being, and improving the quality of life for individuals coping with complex life transitions.

Work Environment

This position offers a fully remote work setting, allowing you the flexibility to manage your schedule while ensuring you provide essential services to clients in need. Our hospice center operates Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours depending on client requirements. On-call support may be necessary for urgent situations, demonstrating our commitment to ongoing care and responsiveness to client needs.

Key Responsibilities
  • Conduct comprehensive assessments of individual clients' psychosocial needs, focusing on emotional, mental, and social facets of their well-being.
  • Collaborate closely with seniors and their families to develop individualized care plans that prioritize their personal goals and preferences, ensuring tailored support.
  • Provide counseling and emotional support that assists clients in navigating challenging life transitions, such as loss of independence, health declines, and bereavement.
  • Advocate for clients' rights and assist them in accessing community resources, ensuring they receive the necessary support and services to thrive.
  • Engage with interdisciplinary teams, including nurses and caregivers, to coordinate holistic care that addresses both medical and psychosocial aspects of patient health.
  • Conduct regular home visits to monitor clients' well-being, inspect living conditions, and mitigate any concerns regarding safety or care.
  • Facilitate open communication among family members to resolve any conflicts, fostering a collaborative approach to caregiving.

Qualifications
  • A Master's degree in Social Work from an accredited program is required.
