Work Environment

This position allows for remote work, providing flexibility while ensuring that our social workers maintain a high level of connection with clients. Our innovative approach facilitates a collaborative team atmosphere where your contributions are valued. Full-time work hours will be from Monday to Friday, 9:00 AM to 5:00 PM, allowing for a consistent schedule that supports work-life balance.

Key Responsibilities

• Act as a liaison between families and outpatient providers to ensure comprehensive care coordination.
• Engage with insurance companies to facilitate pre-certification and manage utilization effectively to support patient needs.
• Contribute actively to discharge planning and aftercare processes, ensuring a smooth transition for clients.
• Provide therapeutic meal support, working to model healthy relationships with food and tackle issues related to eating and nutrition.
• Deliver empathetic and affirming care tailored to each client's unique circumstances, respecting their intersecting identities.

Patient Population / Client Focus

In this role, you will primarily engage with diverse client groups, including older adults who may be facing challenges related to mental health, social isolation, and other age-related issues. Your expertise will help address their unique concerns and support their journey toward improved well-being.

Qualifications

• Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Colorado is required.
• Proven experience working with older adults is essential.
• Strong clinical and diagnostic skills to effectively assess and manage various mental health conditions.
• Exceptional communication and interpersonal skills to foster trust and rapport with clients.
• Ability to work independently, as well as collaboratively within a remote team framework.

Preferred Experience

• Experience with community health clinics or similar healthcare facilities.
• Familiarity with electronic health records (EHR) systems to document assessments and treatment plans efficiently.
• Knowledge of current mental health practices and guidelines, coupled with a commitment to continuous learning.

Compensation and Benefits

The expected salary for this position ranges from USD 90,000 to 105,000 per year, based on experience and qualifications.
