• Advocate for clients in accessing community resources, social services, and support systems to ensure their holistic well-being during their hospice journey.
• Provide immediate support and crisis intervention, employing evidence-based techniques to stabilize clients during moments of distress.
• Maintain meticulous records of client interactions, treatment plans, and progress notes, ensuring compliance with both HIPAA regulations and ethical standards.
• Engage in case consultations with an interdisciplinary team to ensure all aspects of care are being met collaboratively and effectively.
• Educate clients and their families about coping mechanisms, self-care strategies, and effective communication tools to empower them during this challenging time.
• Participate in community outreach initiatives, and build partnerships with local organizations to expand referral resources for clients and families.

The clients served in this role primarily consist of individuals facing terminal illnesses, along with their families. The focus is on providing compassionate care that addresses not just their physical but also their emotional, psychological, and social needs. As a Licensed Clinical Social Worker, you will be instrumental in helping clients process their experiences, express their emotions, and develop coping strategies that promote a sense of peace and dignity during the final stages of life.

• A current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• A Master’s Degree in Social Work (MSW) or a closely related field is required.
• A minimum of four years of practical experience in social work, specifically within mental health domains.
• Previous experience in an outpatient or hospice care setting is strongly preferred.
• Exceptional communication and interpersonal skills that facilitate trusting relationships with clients and families.
• A solid commitment to ethical practice and confidentiality in all client interactions.
• The ability to work autonomously while actively participating as a collaborative member of a team.
