• Conduct thorough intake assessments to identify clients' mental health needs and challenges.
• Design and implement personalized treatment plans that address specific client goals, including coping with grief, anxiety, and end-of-life issues.
• Provide individual counseling sessions through various mediums, including video conferencing and phone consultations, adapting to client preferences.
• Track client progress by developing detailed progress notes, treatment summaries, and case reports that inform ongoing treatment decisions.
• Engage in crisis intervention strategies as needed, ensuring clients receive immediate support during critical moments.
• Foster collaborative relationships with healthcare professionals within the hospice team, facilitating a multidisciplinary approach to client care.

This role focuses on a diverse population, primarily consisting of patients in hospice care who may face unique emotional and psychological challenges as they navigate their end-of-life journeys. The LCSW will be responsible for offering compassionate support to clients dealing with grief, loss, trauma, and related mental health concerns, ensuring they receive the emotional care tailored to their specific circumstances.

• Master’s degree in Social Work from an accredited institution.
• Current Licensed Clinical Social Worker (LCSW) credential in Massachusetts.
• Proven experience in delivering mental health services, particularly with hospice clients or similar vulnerable populations.
• Strong diagnostic skills and the ability to treat a wide range of mental health disorders, with a focus on effective interventions.

We offer a competitive salary ranging from $70,000 to $90,000 per year, commensurate with experience and qualifications.
