• Directly provide clinical services to clients experiencing symptoms of major mental illness and severe emotional disorders, both in individual and group settings.
• Develop and implement integrated care plans that outline treatment goals and link clients to additional necessary services.
• Conduct developmentally appropriate screenings and assessments, utilizing evidence-based practices to address client needs effectively.
• Facilitate crisis intervention efforts, offering support and strategies to manage acute emotional distress as needed.
• Maintain a caseload of clients, ensuring timely and thorough documentation, progress assessments, and treatment plan updates.
• Participate in interprofessional team meetings to coordinate care and share insights on client progress and program effectiveness.
• Monitor safety and well-being by identifying potentially hazardous situations and taking appropriate action to mitigate risks.
• Provide education to clients and families about mental health resources and support systems available within the community.
• Contribute to agency service delivery committees to enhance quality improvement initiatives.

The clients you will work with primarily include individuals facing severe emotional and mental health challenges, often requiring tailored support in the context of hospice care. This role is critical in providing compassionate interventions that not only address immediate needs but also empower clients and their families to navigate their healthcare journey effectively. Your commitment to culturally competent care will ensure that clients from diverse backgrounds feel understood and respected.

• A Master's degree in Social Work (MSW) from an accredited institution.
• Active LCSW license in New York State.
• Demonstrated expertise in assessment, diagnosis, risk assessment, and crisis management, specifically within hospice or similar healthcare environments.
• Strong verbal and written communication skills, with an ability to organize and prioritize tasks efficiently.
• Experience working with diverse populations and a commitment to understanding their unique cultural and personal perspectives.

• A minimum of 2 years of clinical experience in a healthcare setting, with a focus on mental health and hospice care preferred.
• Familiarity with evidence-based practices in social work and mental health interventions.
• Experience with electronic health records (EHR) systems and data management related to client care.

This position offers a competitive salary ranging from USD 70,000 to 90,000 annually, commensurate with experience.
