Nurse Practitioner (NP) / Lead Nurse Practitioner – Telehealth (Arizona)

Remote | Medicaid Population | Value‑Based Care

About the Role Join a new telehealth‑focused clinical team caring for complex Medicaid populations across Arizona. Work remotely with an interdisciplinary team to improve outcomes, reduce unnecessary utilization, and support vulnerable patients. Lead NP roles available for candidates with strong leadership experience.

Key Responsibilities

  • Provide autonomous NP care with prescribing authority
  • Lead or support interdisciplinary care coordination
  • Deliver virtual care and remote patient management
  • Coordinate with PCPs, specialists, and health plan case managers
  • Support proactive care management and early risk identification
  • Provide 24/7 clinical access to reduce avoidable ER visits
  • Assist with hospitalization support and post‑discharge transitions
  • Contribute to building a new value‑based care model

Schedule & Employment Options

  • Primarily remote telehealth
  • W‑2 or 1099 options available

Compensation

  • $130,000–$175,000 base, depending on experience and Lead NP designation

Required Qualifications

  • Master's degree or higher
