Job description

Join the inpatient Care Management team at Rice Memorial Hospital in the role of Social Worker. 

Summary

  • Our Social Workers at the Rice Memorial Hospital serve patients and families in our community and beyond by helping them during a challenging time in their lives. 
  • Enjoy the variety of cases they are presented with as there is always something new and different. 
  • Collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. 
  • Social Workers have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.

We will provide supervision for new grads in-house or will reimburse up to $175/month for external supervision. 

Schedule

  • Full-time | 64 hours every two weeks | Onsite
  • Day shift | Weekdays, typically 7:30AM to 4:00PM | Varied schedule, 4 days per week

Pay based on experience and degree.  Full range Pay range: $30.65 - $45.89 per hour

Qualifications

  • Current Licensed Social Worker (LSW) in Minnesota required. 
  • Bachelor's or Master's degree in Social Work. 
  • Prefer minimum of one year of experience as a Social Worker (not required).
  •  Prior experience in a healthcare setting preferred (not required). 

To be eligible for the hiring and relocation bonus you must be an external candidate who is hired into an eligible full or part-time benefited position.  Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.  To be eligible for the relocation bonus, you must currently reside more than 50 miles away from St. Cloud Hospital and relocate to within 30 miles of it. CentraCare hiring incentives to include but not limited to, hiring and relocation bonuses may be discontinued at any time.
