Work Environment

This position is primarily remote, allowing for flexibility in your work schedule while you engage with clients from the comfort of their homes or our community health clinic. Key Responsibilities
  • Conduct comprehensive bio-psychosocial evaluations to identify individual patient needs and barriers to treatment, developing personalized treatment plans that align with their mental health goals.
  • Utilize a variety of therapeutic modalities, including individual and group therapy, to address the psychological needs of patients, monitoring their progress and adapting treatment strategies as necessary.
  • Consult closely with our multidisciplinary team, providing insights into the psychosocial factors affecting our patients and participating in collaborative care meetings to enhance treatment compliance and coordination.
  • Document all social work activities in accordance with departmental policies, maintaining accurate and timely patient records that reflect each patientโ€™s journey and progress.
  • Educate patients and their families about psychosocial recommendations and medical advice, facilitating a clear understanding of treatment processes to boost compliance and engagement.
  • Advocate for patients within the community by coordinating support services, with a focus on self-determination, thereby improving social support networks and resource access.
  • Engage in continuous professional development through training and educational opportunities, staying abreast of current community resources to enhance patient outcomes.
Patient Population / Client Focus

The primary focus of this role is on patients from diverse backgrounds who are facing various psychosocial challenges, ranging from mental health disorders to situational crises. Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited graduate institution.
