POSITION SUMMARY
The Regional RN Care Coordinator ensures that resident care needs aremet by coordinating and ensuring assessments are completed and all residentshave accurate service plans at assigned locations as requested by regional or community leadership. Provide training and education of staff in all areas of health-related services,policies and procedures as assigned. The Regional RN Care Coordinator supportsmultiple communities, providing RN services to include on-call support andphone triage, nursing assessments, evaluating and documenting significantchanges in condition, performing wound assessments, and conducting on-siteroutine clinical reviews.
ESSENTIAL DUTIES AND RESPONSIBILITIES
  1. Completesor oversees nursing assessments required for significant changes of condition,in accordance with applicable regulations. Assesses health, functional and psychosocial status of residents.Participates in the development of individualized service plans
  2. Supports multiple communities as assigned, providing RN oversight and ensuring timely completion of requirednursing evaluations and documentation. Responsibilities include phone triage,on-call support, wound assessments, significant change evaluations, and routineclinical reviews
  3. Provides training of staff in health-related services including but not limited to providing personal care and medication management
  4. Overseesthe documentation and communication of resident care and services. Assists themanagement team to ensure resident records are completed in a timely manner and maintainedaccording to company policies and state regulations
  5. Delegatingnursing tasks may be required, as permitted under applicable state nursing lawsand practice acts, with appropriate communication maintained with residents, their families, and healthcare practitioners
  6. Maintains and oversees medication services including staff training, staff oversight, pharmacy coordination, medication room and medication systems
  7. Maintains up-to-date knowledge of all rules, regulations and policies pertaining to resident care. Staysinformed of industry and health care trends and standards, applying newknowledge to meet the changing needs of residents. Seeks out additionalexpertise and advice when needed to assure residents' needs are met and properpractices followed
  8. Performs other duties as assigned

QUALIFICATIONS AND EXPERIENCE
Must have current, unencumbered RNlicense in the state of practice. Experience in wound assessments, coordination of care, weight monitoring and interventions as needed, identifying and troubleshooting shooting for a change of condition required. Experience in geriatric nursing,medication administration, case management, and training and staff developmentpreferred.
Delegation experience in the state assigned preferred.
EDUCATION
Registered Nurse license and Bachelor of Science in Nursing or Associate Degree in Nursing.
