REFERRAL AND INTAKE NURSE (LPN)


At Unity Hospice, we bring peace of mind and comfort to patients and families when it matters most. We're growing and looking for a Referral & Intake Nurse (LPN) to join our team and serve as a key connection point for patients, families, and referral partners across all Unity territories.


This role is ideal for someone who thrives in a fast-paced environment, enjoys coordination and communication, and is passionate about supporting patients at the very start of their hospice journey.


Work environment: Remote


Schedule:  Part-time (3 days per week, 24 hours per week)  8:00 a - 4:00 p or 9:00 a - 5:00 p,, working every 4th weekend, 1-2 holidays per year. 


What You'll Do:
As the first point of contact, you'll play a vital role in ensuring a smooth and compassionate referral process. Responsibilities include:

  • Managing incoming referrals and gathering patient medical information 
  • Communicating with hospitals, clinics, and care teams 
  • Coordinating initial visits with patients, families, and admissions staff 
  • Verifying insurance coverage and obtaining necessary orders 
  • Supporting eligibility review and following up on pending referrals 
  • Maintaining accurate and timely documentation

1. Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin

2. Valid WI Drivers' License, reliable transportation, and proof of current automobile insurance coverage
