We are seeking a highly motivated and experienced ReEntry Case Manager to join our team. The ideal candidate will have experience in case management, juvenile justice systems, and working with youth on probation or parole. This position is based in Portsmouth, VA but travel throughout the region is required.


Work involves:

  • Being assigned youth before their release from custody and remaining with them through the entire reentry process.
  • Developing individual plans of care and delivering a wide array of support services to youth to include educational, vocational, employment opportunities, etc.
  • Prepares records of client visits, progress, and case notes.
  • Responds to emergencies and crisis interventions as needed.
  • Establishes working relationships with families, court personnel, referral agencies, and other service providers.
  • Travel in own vehicle is required.
  • Flexible schedule is required including weekends, weekday evenings and especially daytime hours.
  • Mileage reimbursed.

Preferred: Bachelors degree from an accredited college or university with human services, criminal justice, or other relevant degree fields and 1-3 years of case management experience that may be paid, volunteer, internship or practicum are preferred.
