Job Details

Responsibilities include:

Coordinates efforts of caseloads to ensure high quality programming and ensure regulatory compliance for individuals with mental illness.  Assists program manager in all aspects of daily operation of the Horizon PROS facility.

  • Program recovery oriented activities in a group setting based on the individual action plans
  • Completes all required documentation for billing within agency guidelines.
  • Acts as liaison to outside agencies, ensuring continuity of services
  • Responsible for coordination of individuals care through the recovery process.
  • Provides individual counseling as needed.
  • Assists individuals in developing their Individual Action Plan defining goals that are recovery oriented.
  • Assists individuals in crisis using their Wellness Recovery Action Plan, assists individuals in crisis and monitors individual safety plans and AOT orders
  •    In absence of Program Supervisor, may be asked to assist with building responsibilities. 
  •   Assist with program re-certification process, through quality assurance reviews.
  • Ensures written documentation is of professional quality and   is forwarded to outside agencies in a timely manner.          

Qualifications:

  • Bachelor's degree or CASAC certification required.One year experience in the field required, 3 years experience preferred.
  • Maintains valid NYS driver's license within agency criteria

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
