MEDICAL RECORDS Assistant PRN as needed

DEPARTMENT:      NURSING

RESPONSIBILITY: Medical records Director

JOB    SUMMARY: The purpose of this position is to maintain and store the medical records of all residents according to state and federal regulations.

QUALIFICATIONS:

            1.         A high school diploma.

            2.         Trained or experience in the processing of medical records is preferred.

            3.         Must be able to read, write, and speak English in an understandable manner.

JOB REQUIREMENTS:

            1.         Must be physically and mentally capable of performing routine job duties.

            2.         Good managerial, organizational, and planning skills.

3.         Be personable with residents, families, and staff in a professional and         cooperative manner.

            4.         Must have compassion, tolerance, and understanding for the elderly.

            5.         Knowledge of JCAHO, OBRA, IDPH, and HFS regulations relevant to     

                        medical records.

MAIN DUTIES:

A.        Support the facility's philosophy of care and strive to achieve its goals and objectives.

            B.        Maintain EMR and/or in-house charts according to facility policy.

C.        Make sure all orders or paper forms requiring a physician's signature are signed , dated and scanned into EMR.

            D.        Scan & attach to pertinent resident file any paper documents such as lab results,

                        x-rays, consultant reports, etc.

            E.         Maintain medical record room in a neat and orderly fashion. Ensure all

                        equipment such as copier, scanner are in working order.

F.         Maintain file of all paper records of discharged and deceased residents.

                        Arrange for medical records destruction for paper records which no longer need

                        to be maintained.

G.        Notify Administration of any record request. Copy, print or download on disc

                        resident’s record as requested.

H.        Maintain the confidentiality of resident information and honor his/her personal and property rights

I.          Follow established fire, disaster, safety, infection control and evacuation policies and procedures.

J.          Perform other related duties as directed by the DON and Administrator.

K.        Admission agreement duties
