Summary:
The Patient Flow Coordinator Team lead, under the direction of the Manager, Patient flow Center, will be responsible for working as a patient flow coordinator with these additional duties:Oversee the patient bed management queue, documentation, and staff assignments.Ensure the handling of bed placement workflows are followed Calling staff to cover for absences: If cannot find coverage may require working in addition to regular scheduled work time. Provide support to patient flow staff and in challenging situations, assist and take the lead when placing emergent patients and/or patient bed management issues that need escalation.Assist with educating & reeducating team members regarding new workflows, procedures, and practices.Address issues as they arise professionally and if unable to resolve escalate to Manager, Patient Flow or Manager on call for clinical command centerReport any concerns involving the patient flow center to Manager, Patient Flow or manager on call for clinical command centerAttempt to deescalate volatile situations within the call centerMonitor and control noise level inside the brain where multiple teams workHelp promote teamwork and boost moraleWork closely with Transfer Center, Comms Center, facility leadership, Behavioral Health and LTC while on shift to place patients as timely as possibly"air traffic controls" the management of patients beds for all Ballad facilities during shift

The Patient Flow Coordinator coordinates patient throughput from all points of entry to facilitate overall hospital patient flow by collaborating with the medical staff and other members of the interdisciplinary team in the coordinating, organizing, and planning, of patient movement within the system. The coordinator will be responsible for adding and accepting patients to appropriate beds based on patient acuity level, services needed and hospital capacity restraints. This person will coordinate and strategize the complex placement of incoming ER and direct admissions, inter-hospital transfers, post-operative patients needing inpatient and patient transfers outside of the system in accordance with Ballad policies, using real-time capacity planning dashboards. The Patient Flow Coordinator will communicate ongoing changes and challenges to the Patient Flow Clinical Lead or the Manager. He/she will work with house supervisors, facility CNO's, transfer coordinators, and RCMCC coordinators to manage patient admissions, transfers and placement within the capacity and capabilities of each facility/unit while continually air traffic controlling the system capacity and needs. This position will utilize his/her knowledge and expertise to facilitate safe and efficient patient throughput focusing on placing the right patient in the right bed at that time. Accurately relay information regarding diagnosis and special needs. Monitor bed planning performance, managing supply and demand. Engage in continual problem solving to maintain positive patient flow under tight time frames and limited bed resources: multitask according to changing priorities. Collaborate with EVS to manage and prioritize patient room cleaning. Prepare for daily bed meeting and assess for admission capacity by communicating with relevant House Supervisors in a timely manner; assess need for overflow plan. The Patient Flow Coordinator will prepare end of shift report.
The coordinator will maintain a professional image and exhibit excellent customer service and relations with physicians, nurses, and co-workers. The coordinator will demonstrate leadership and commitment with respect to the quality management system by promoting the use of the process approach and risk-based thinking by engaging, directing, and supporting persons to contribute to the effectiveness of the quality management system requirements by promoting improvement. The Patient Flow Coordinator monitors real time throughput metrics and escalates delays in patient flow to clinical coordinator and/or the manager, patient flow center. The coordinator will maintain real time accuracy of bed placement software, including but not limited to patient demographic data and patient placement indicators. Utilize the chain of command for clinical or other situations that are beyond the scope of the placement specialist immediately to support flow and the safety of patient placement.
The Patient Flow Coordinator will ensure accurate and concurrent data collection and maintenance in the electronic health record. The coordinator will complete training requirements and demonstrates competence in all software applications needed to effectively perform job duties. The coordinator will adhere regulatory requirements such as EMTALA, Ballad Health Ethics and Compliance policies, and quality initiatives. As well as to monitor self-compliance and implements process changes to ensure compliance to regulations and quality initiatives. The coordinator ensures high quality, patient-centered care by facilitating the throughput of patients within the entire health system to provide optimum efficiency of resources and timely management of patient care.
Requirements:
Bachelor's Degree - RN or LPN with 5 years experience in clinical environment.
