Job description

JOB TITLE: Quality Improvement Coordinator
STATUS: Per Diem
LOCATION: 50 Leroy Stree, Potsdam, NY 13676
DEPARTMENT: Patient Safety & Quality
SCHEDULE: 8am-4:30pm
SUMMARY
Guides leadership in clinical quality-based on data gathered, in conjunction with the Performance Improvement Specialists. Performs duties in assigned key process areas as assigned. Analyzes and reports data to the appropriate affiliate's departments and committees.
RESPONSIBILITIES
  • Primary support to the Administrative Leadership Team for implementation of departmental quality/safety plans, which includes Peer Review, Mortality and Morbidity reviews and annual quality reports. Assists the Institute for Patient Safety in the implementation of departmental quality/safety plans, programs, and annual reports.
  • Performs chart audits, evaluates hospital activities and identifies risk through the analysis of quality referrals and complaints to identify trends and opportunities for improvement.
  • Collaborates with Risk management as needed.
  • Makes appropriate referrals to department directors/managers and/or Senior Leaders regarding quality, safety and risk management issues.
  • Supports and participates in departmental, agency, and system committees and project teams.
  • Assists in educating staff in assigned clinical/department areas on appropriate use of recognized quality improvement tools, patient safety and clinical outcome measures, and key processes.
  • Disseminates quality improvement information to pertinent departments and committees.
  • Support and assist in implementing mechanisms to ensure participation of all staff in the reporting of potentially harmful conditions or behavior in a "Just Culture" environment.
  • Assists Director in assessing survey readiness of all departments/services and provides consultation where indicated.
  • Provides administrative and staff updates and education regarding any regulatory or standards changes.
  • Performs other duties as assigned.

MINIMUM QUALIFICATIONS
  • 3 or more years in Quality/Patient Safety or Bachelor's degree in Engineering/IT/Business/Finance/Healthcare or Nursing required;
  • Master's preferred
  • Minimum of three to five years of previous Health Care experience.

PHYSICAL REQUIREMENTS:
