Position Overview
Devoted Care Hospice is seeking an experienced and detail-oriented Licensed Vocational Nurse (LVN) with hospice experience to serve as our QAPI Nurse.
This role is essential to maintaining regulatory compliance, supporting clinical excellence, and ensuring interdisciplinary documentation consistently meets Medicare Hospice Conditions of Participation (CoPs) and payment eligibility requirements. To strengthen audit credibility and ensure quality review remains grounded in real-time clinical practice, this position will also provide occasional clinical field support to RN Case Managers during periods of lower audit volume. Maintaining active patient care involvement allows the QAPI Nurse to align documentation standards with current bedside practice and reinforces clinically informed compliance oversight.
The ideal candidate combines strong hospice clinical knowledge with exceptional documentation review skills, attention to detail, and a commitment to quality improvement.
Primary Responsibilities
QAPI & Compliance Oversight
  • Perform detailed review of interdisciplinary documentation including:
    • Nursing
    • Social Work
    • Chaplain
    • Hospice Aide
    • Therapy services (as applicable)
  • Ensure documentation consistently supports:
    • Hospice eligibility and continued appropriateness
    • Clear and defensible decline trajectory
    • Symptom burden and terminal prognosis
    • Compliance with Medicare Hospice Conditions of Participation
  • Identify documentation gaps impacting regulatory compliance or reimbursement
  • Provide real-time feedback and documentation coaching to clinical staff
  • Assist with chart audits, ADR preparation, and survey readiness activities
  • Review certifications, recertifications, and supporting clinical narratives for accuracy and compliance
  • Track documentation trends and participate in agency Quality Assurance and Performance Improvement (QAPI) initiatives
  • Collaborate with clinical leadership to maintain continuous survey readiness
  • Initial review and ensuracnce of HOPE compliance.

Clinical Field Support
  • Provide occasional patient visits during downtime hours to support RN Case Managers
  • Assist with routine hospice visits within LVN scope of practice
  • Reinforce documentation best practices during clinical care delivery
  • Support continuity of care and interdisciplinary collaboration
  • Serve as a clinical resource promoting hospice standards of care

Qualifications
  • Active Texas LVN license in good standing
  • Hospice experience required (minimum 12 years preferred)
  • Strong understanding of:
    • Hospice eligibility criteria
    • Documentation supporting terminal prognosis
    • Decline trajectory and hospice appropriateness
    • Medicare Hospice Conditions of Participation
  • Experience in chart auditing, compliance review, or quality improvement preferred
  • Strong organizational, analytical, and critical thinking skills
  • Ability to work independently while collaborating with interdisciplinary teams
  • Excellent written and verbal communication skills
  • EMR proficiency required (WellSky/Kinnser experience preferred)
