Job description

Position Description: SUMMARY: Assist in working with frail and disabled adults. Contribute to the improvement or maintenance of the physical, mental, and emotional well-being of the program participants. ESSENTIAL DUTIES AND RESPONSIBILITIES:1. Assist participants with feeding, mobility, and toileting.2. Provide personal attention to the participants.3. Assist with daily therapeutic activities. 4. Assist with exercise and walking programs.5. Participate with other staff in the development of each participants individual treatment plan, evaluation of participant progress, and planning for discharge of participant.6. Track care plan progress and outcomes using simple computer software and documentation.7. Share any special skills and talents. 8. Help maintain a clean and safe environment. Assist with setting up and taking down furniture as directed.9. Other duties as assigned.RELATIONSHIPS:1. Supervised by the ADHC Program Manager2. Works as a member of the ADHC team and consults with the Activity and Nursing Coordinators when needed.SUPERVISORY RESPONSIBILITY:This job has no supervisory responsibilities.QUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.Education and/or Experience:1. High School diploma or GED.2. Experience or training in working with frail, disabled older adults.3. Basic computer skills are preferable.4. Nursing assistant or home care aide experience preferable.Other Skills and Abilities:1. Be willing to obtain a TB test, CPR training, and Food Handlers Permit.2. Physical ability to assist disabled participants with transferring and ambulating; must be able to lift and move 50 lbs.3. Ability to work with people of diverse economic and social backgrounds and lifestyles.Note: Employees are subject to periodic WA State Patrol Criminal History Background Checks and must complete a Criminal History Self-Disclosure Form.LANGUAGE SKILLS: Ability to speak effectively with clients, including those who are hard of hearing, and read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. MATHEMATICAL SKILLS: Basic math skills: addition, subtraction, multiplication, and division.REASONING ABILITY: Ability to apply common sense understanding to carry out instructions and to problem solve.PHYSICAL DEMANDS: While performing the duties of this job, the employee is regularly required to sit, talk and hear.
