Job description

As a Permanent Supportive Housing Program Coordinator, you’ll play a vital leadership role in guiding compassionate, effective services that help people achieve lasting housing stability and self-sufficiency.

We’re seeking candidates who bring either a Bachelor’s degree in Social Work or a related field, or at least four years of case management experience supporting low-income individuals and families—particularly those experiencing housing instability. A combination of education and relevant experience that demonstrates strong leadership and a deep understanding of poverty-related challenges will also be considered.

In this role, you’ll collaborate closely with the Program Manager and Program Director to oversee services across Rock and Walworth Counties. You’ll lead and support case management efforts, ensuring high-quality, client-centered care while maintaining program integrity. Most importantly, you’ll empower participants to build the skills and confidence needed to secure stable housing and move toward greater independence.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
  • Meet facetoface with participants at a frequency as required by program funder through home or office visits.
  • Complete required forms, paperwork and documentation associated with operation of program services.
  • Perform data entry of required participant information into HMIS and CAP60 in a timely manner.
  • Provide support and assistance to other housing staff as needed.
  • Review and make recommendations to Program Manager regarding participant related issues involving disciplinary actions, termination, program policy exceptions and consumer complaints.
  • Assist the Program Manager in completion of required reports.
  • Coordinate participant program services with other internal and external service providers.
  • Develop and maintain positive working relationships with funding sources, community partners, participants, and peers.
  • Attend staff, department, agency, and local community meetings as required.

  • Followup on all program service referrals/requests as assigned.
  • Complete related work as assigned within the scope of responsibility and skills required for the job.

  • MATHEMATICAL SKILLS:
  • Knowledge of basic math: add, subtract, multiply, divide, fractions, and percentages.
  • REASONING ABILITY:
  • Ability to interpret a variety of instructions in written, oral or diagram form, to solve problems requiring decisionmaking skills within department guidelines.
  • CERTIFICATES, LICENSES, REGISTRATIONS:
  • Current social work certification from State of Wisconsin preferred.
  • Access to reliable, licensed, insured driver and transportation required.
  • HQS certification preferred.
  • OTHER SKILLS AND ABILITIES:
  • Must possess strong interpersonal, organizational, and independent work skills.
  • Ability to work independently and collaboratively with diverse staff and participants and provide services in a structured, yet individualized, creative, and nonjudgmental manner.
  • Must possess experience with office equipment and procedures and basic budget and financial transaction procedures.
  • Must have computer background and experience with Microsoft applications and internet.
