Job Description:

The Typical Life Corporation (TLC) Incident Management Coordinator will fulfill the role of Certified Investigator within the Incident Management Department. This position reports to the Director of Incident Management.

At TLC, the care of our individuals and staff is our highest priority. The Incident Management Coordinator plays a big role in this, ensuring that we maintain a safe and healthy environment.

The Incident Management Coordinator investigates issues of concern and ensures that all assigned investigations are completed following the guidelines of timeliness, thoroughness, and objectivity. The Incident Management Coordinator must display a comprehensive knowledge of the Guidelines for Incident Management and Investigations and utilize critical-thinking skills in managing the workflow and decision-making responsibilities within this role. This person should also be capable of providing quality improvement recommendations and risk management and trend analysis as defined by ODP.

The Incident Management Coordinator is responsible to:

· Ensure that all Certified Investigations assigned and completed follow the guidelines of Timeliness, Thoroughness, and Objectivity.

· Report immediate concerns which arise during an investigation to the Director of Incident Management.

· Acts as and fulfills all duties of the TLC point person when designated by the Director of Incident Management or assigned On Call point person duties.

• Responsible to ensure all assigned incidents and investigations are entered into the Enterprise Incident Management (EIM) system within the required 24- or 72-hour timeframes.

• Ensure that all assigned Administrative Review and Final sections are completed within 30 calendar days, or an extension is filed, as necessary.

• Ensure that EIM completed incidents meet guidelines as set ODP.

• Communications with County AE, other Agencies such as Adult Protective Services (APS) and ODP representatives regarding incident management are made when required.

· Ensure that all assigned Investigation files are complete, securely maintained, and readily available for review by the Director of Incident Management and oversight entities as requested.

· Participates in Quarterly Certified Investigator Peer Review process.

• Ensures that Individuals, families, or persons designated by the individual are notified of investigations, as well as results of the investigation upon completion.

• Identify and support the implementation of individual and systemic changes based on quality and risk management analysis to mitigate individual risk, mitigate agency-wide risk and promote the health, safety, and rights of all individuals.

• Support individuals, teams, and programs to successfully implement aspects of the quality management plan, incident management, and risk mitigation procedures.

• Will contribute to the monthly, quarterly, and annual Quality Reports as required by appropriate leadership or board membership, but no less frequently than quarterly.

• Will be responsible for assisting in the education of all appropriate personnel in TLC Policies and Procedures as well as any Bulletin affecting the Investigatory or Incident Management process.

• Shall work with other members of the IM/Quality team to develop best practice methodologies for Investigations, Incident Management, Administrative Review, and implementation of Corrective Actions.

Qualifications Include:

· High School Diploma of GED

· Minimum of 3-5 years’ experience working with MH/IDD population

· Must not have any confirmed allegations of abuse or neglect

· The person must be a certified investigator (CI) or be able to obtain this certification within 90 days of hire
