Family Nurse Practitioner/Physician Assistant
Crossroads Healthcare Clinic, a program of Community Action of Laramie County, is seeking a full-time Family Nurse Practitioner and/or Physician Assistant. The Family Nurse Practitioner/Physician Assistant works independently under the administrative supervision of the Director of Crossroads Healthcare Clinic. The function of the Family Nurse Practitioner/Physician Assistant is to provide primary health care services to individuals and families of all ages who are homeless or low income in the Laramie County area. This postion provides services in a traditional healthcare setting. The Family Nurse Practitioner/Physician Assistant is responsible for providing healthcare services that facilitate the uniform, efficient, and effective clinical operations at the Crossroads Healthcare Clinic. In coordination with the Crossroads Healthcare Clinic Program Director, this position also oversees the quality assurance and improvement process including the clinical oversight of licensed or registered medical staff employed by the agency.
Job Responsibilities:
- Delivers direct primary care to clients of all ages by developing therapeutic plans of care for prevention, management and maintenance of optimum health for individuals.
- Assesses, diagnoses, treats, manages, evaluates and monitors acute and chronic illnesses, and administers prevention services.
- Orders, performs, interprets, evaluates, and reviews diagnostic studies and procedures.
- Prescribes pharmaceuticals, evaluates outcomes and dictates changes in management levels of therapeutics.
- Provides health education and counseling to promote positive health behaviors and self-care skills.
- Makes referrals to health care professionals, specialists, other clinics, and community agencies.
- Maintains client records in the electronic health record system in a systematic format approved by the Program Director.
- Serves as a consultant and collaborator to/with the other community health centers, clients, and community professionals.
- Participates as a preceptor and role model for Registered Nurse, Nursing and Medical Assistant students.
- Assists/instructs other health care employees in the management of client care.
- May be involved in interviewing and selection of health care employees.
- Ensures that client records are kept confidential consistent with the Crossroads Healthcare Clinic policies and procedures and HIPAA standards. Monitors HIPAA compliance within the work environment and reports concerns and/or breaches to the Program Director immediately.
- Provides well-childcare, including screening and immunization in accordance with State regulations.
- Demonstrates knowledge of OSHA regulations and assists the Director to ensure safety in the clinic setting.
- May be asked to, under the auspices of the Program Director, to supervise subordinate staff to include: prioritizing and assigning work; assess clinical skills for performance evaluations for clinical staff; ensuring staff is trained; and making recommendation for hiring, disciplinary action and termination.
