Department: La Crosse Care Center             

FLSA Status: Non-Exempt

Position Summary

The La Crosse Care Center RN will provide services to our clients, striving to provide these services in a compassionate, professional, ethical, and knowledgeable fashion. The RN role will include but are not limited to: the assessment, planning, implementation, and evaluation of the patients’ status, plan of care.

Essential Functions

  • Responsible for providing direct supervision and program implementation of Care Center medication, Health assessment and related care protocol’s
  • Assesses & evaluates nursing & medical needs of patients who are admitted (e.g., admissions, ongoing treatment, discharges);
  • Provides professional nursing care (e. G., administers medications & treatments as ordered by physician, educates patient on medication(s), assists with physical examinations, takes & records vital signs);
  • Responds to emergency situations, intervenes and defuses, when possible, psychiatric emergencies within the facility. (e.g., codes, suicide, assault).
  • Observes, evaluates & records patient behavior, condition & response to medication, treatments & therapeutic activities;
  • Participates in ongoing performance improvement/quality assurance.
  • Transcribes &/or verifies doctors' orders;
  • Prepares, reviews, implements & updates records/reports/plans (e.g., shift reports, transfers, admissions & discharges);
  • Serves as interdisciplinary treatment team member;
  • Performs variety of miscellaneous duties e.g., light housekeeping in patient areas & living quarters; Identifies & reports safety concerns;
  • Case management responsibilities associated with the role of Psychiatric RN, including intake and level of functioning assessments, mental status examinations, and discharge planning.
  • Assist in coordination of mental health treatment between Psychiatrists and other practitioners.
  • Assist in evaluation and acceptance of, as well as provide, referrals to/from outside agencies and facilities.
  • Document all patient contacts.
  • Verify past patient mental health history, including verification of any psychotropic medications used.
  • Participation in overall program development and management, including quality improvement, staff training and interface with other levels of mental health services.
  • Maintain confidentiality of client medical records.
  • Communicates and collaborates with all team members, patients, and families effectively. Ensures patient care needs are met in a timely manner and follow through is initiated and/or completed. May act as a patient advocate.
  • Maintains thorough and complete documentation according to established legal guidelines. Transcribes and inputs physician orders accurately and in a timely manner.
  • Performs all aspects of clinical practice according to standards of care, including, but not limited to: assessment, implementing, evaluation, intervention, organization of work, decision making, follow-up, activities of daily living,
  • Perform other duties and responsibilities as delegated.

Qualifications

  • Current Registered Nurse in the state of Wisconsin
  • Current with nursing skills, equipment, and unit specific knowledge
  • 3 or more years of experience
  • Proficient with MS Office programs, EMR, internet, and email
