Job Description: Care Coordinator
Position Overview:
BeeHive Homes is an assisted living facility taking care of elderly residents. We are in need of a new Care Coordinator in one of our homes. Our Care Coordinators help manage the day to day functions of the home. If you are compassionate, positive, have strong leadership skills, and are organized and creative come join our team.
We are seeking a highly motivated and compassionate Care Coordinator to join our team. The care coordinator will be responsible for coordinating care services for individuals, ensuring they receive appropriate and timely treatment, support, and resources. The successful candidate will have excellent communication and organizational skills and should be comfortable working in a fast-paced environment. This role requires understanding and empathy for individuals dealing with healthcare challenges and the ability to advocate for their needs.
Responsibilities:
1. Coordinate Care: Manage and coordinate care services for individuals by liaising between patients, healthcare providers, and other involved parties. Ensure the delivery of coordinated and comprehensive care across various settings.
2. Report Needs to RN: With RN determine individual needs, goals, and preferences. Collaborate with patients and the RN to develop personalized care plans that address physical, emotional, and social well-being.
3. Advocate for Patients: Act as a patient advocate, ensuring their rights are respected, and their voices heard. Represent patients' interests during interactions with healthcare providers, and other relevant stakeholders.
4. Service Coordination: Collaborate with RN and administrators to organize treatments, and follow-ups. Monitor the delivery and quality of services and assist in resolving any issues that may arise.
5. Documentation and Reporting: Maintain accurate and updated records of patient interactions, care plans, assessments, and referrals. Document outcomes with RN to evaluate the effectiveness of care coordination and identify areas of improvement.
Requirements:
- CNA Certification with 2 or mor
- Previous experience in care coordination, case management, managing other employees or teams or similar roles preferred.
- Strong understanding of healthcare systems, processes, and protocols or be willing to learn
- Excellent communication and interpersonal skills to establish rapport and maintain effective relationships with patients, families, and healthcare professionals.
- Ability to handle sensitive and confidential information with discretion and compassion.
- Exceptional organizational skills with the ability to multitask and prioritize tasks efficiently.
- A commitment to patient-centered care, empathy, and a passion for improving the lives of individuals facing healthcare challenges.
