Job Description:
We are seeking a dedicated and compassionate PRN Licensed Practical Nurse (LPN) to join our team. The LPN will be responsible for providing high-quality care to our residents, ensuring their health and well-being, and assisting with daily activities. The ideal candidate will have a strong commitment to patient care and a desire to work in a supportive, team-oriented environment.


Responsibilities:

  • Administer medications and treatments as prescribed by physicians
  • Monitor residents' health and report any changes to the healthcare team
  • Assist and provide direction to residents regarding their personal care needs, including grooming, hygiene, dressing, nutrition, housekeeping, etc.
  • Maintain accurate and detailed records of care and treatment
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Educate residents and their families on health conditions and care plans
  • Ensure a clean, safe, and supportive environment for all residents
  • Other duties that may become necessary, or as directed by your supervisor or Nurse Manager

This position-shifts will vary based on needs. 


Requirements

  • Current and valid LPN license in Illinois
  • Required to work 1 weekend per month
  • Experience in an assisted living or long-term care setting preferred
  • Strong communication and interpersonal skills
  • Ability to work in a team-oriented environment
  • Compassionate, patient, and empathetic approach to care
  • Ability to handle multiple tasks and prioritize effectively
