At Moravian Village, we are dedicated to providing exceptional care in a CMS rated 5 Star facility. We are seeking a compassionate, detail-oriented Admissions Coordinator to join our team and play a key role in welcoming new residents and supporting their transition into our community.


Position Summary:

The Skilled Nursing Admissions Coordinator is responsible for managing and coordinating the admissions process for prospective residents. This role involves working closely with hospitals, physicians, families, and internal teams to ensure a seamless and positive admission experience. The ideal candidate will have strong communication skills, a deep understanding of the long-term care industry, and a genuine passion for helping others.


Key Responsibilities:
  • Manage all aspects of the admissions process, including inquiries, tours, pre-admission assessments, and coordination of admissions paperwork

  • Collaborate with hospital discharge planners, case managers, and family members to assess referrals and determine appropriate placement.

  • Maintain accurate records in accordance with HIPAA and facility policies.

  • Ensure rooms are prepared and staff is informed of incoming residents.

  • Work with the interdisciplinary team to support resident care plans upon admission.

  • Market the facility's services to referral sources and the community.

  • Provide tours and explain facility services to potential residents and families.

  • Track and report admissions metrics and maintain an active admissions pipeline.
