Job description

Job Description: Tapestry Management Providers (TMPs) play a crucial role in delivering comprehensive, in-person clinical support to healthcare facilities. They ensure proactive patient care through the review and discussion of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI) & daily risk reports with facility healthcare professionals, including physicians, nurse practitioners, and social workers.
This is a full-time, hybrid position that combines remote work with weekly travel to assigned skilled nursing facilities in the Los Banos area. Providers are expected to travel within the assigned territory approximately three days with the remaining two days worked remotely.
Key Responsibilities:
  • Conduct monthly in-person visits to assigned healthcare facilities.
  • Assigned to manage between 1 - 10 Tapestry affiliated facilities.
  • Deliver and review patient CCM, RPM, Daily Risk reports and BHI reports with facility staff.
  • Perform Initial Preventive Visits (IPV) for new patients and re-admits Sas required for enrollment of services.
  • Facilitate discussions with medical professionals, ensuring alignment between data insights and patient care.
  • Ensure clinical observations support report accuracy and provide valuable insights into patient health trends.
  • Collaborate with facility staff to enhance proactive care planning and improve patient outcomes.
Qualifications:
  • Licensed nurse practitioner.
  • Experience in chronic care management, remote patient monitoring, or behavioral health integration preferred.
  • Strong interpersonal and communication skills for effective collaboration with medical staff.
  • Ability to travel to multiple healthcare facilities on a scheduled basis.
  • Detail-oriented with the ability to analyze clinical reports and identify critical insights.

Salary: The anticipated annual salary for this role is $135K - $145K with benefits including 401k, PTO, and CME. We also reimburse for mileage since travel is involved. This hybrid position follows a location-based compensation structure. The posted salary range represents the potential pay range. Actual compensation will be determined based on the candidate's primary work location, experience, qualifications, and internal equity considerations, in accordance with applicable pay transparency laws
Pay Range: $135,000 - $145,000 per year
