Location: Fuquay-Varina Health & Rehabilitation Center – Fuquay-Varina, NC
Schedule: Monday – Friday | Full-Time | Flexibility Required

Reports To: Social Worker / Director of Social Services

Fuquay-Varina Health & Rehabilitation Center is seeking a compassionate and organized Social Work & Discharge Planning Assistant to join our team!

In this role, you will work under the guidance of our Social Worker to help residents and their families navigate the discharge process, coordinate care services, and ensure a smooth transition from our facility to home or another care setting. The position does not require a social work degree, but prior experience in a long-term care or skilled nursing facility is strongly preferred.

Role Overview & Responsibilities

The Social Work & Discharge Planning Assistant plays a vital role in supporting our Social Services Department by:

  • Assisting with planning, organizing, and implementing discharge processes.

  • Communicating with residents, families, and the care team regarding care plans and discharge needs.

  • Coordinating transportation, home health services, medical equipment, and other discharge arrangements.

  • Maintaining accurate documentation in compliance with state, federal, and facility guidelines.

  • Supporting residents and families with questions and available resources.

  • Participating in care plan meetings and family conferences.

  • Providing administrative support, such as scheduling, filing, and updating resident records.

  • Upholding resident rights and maintaining confidentiality at all times.

Qualifications
  • High school diploma or equivalent required.

  • Experience in a long-term care, skilled nursing, or healthcare setting preferred.

  • Strong organizational, communication, and customer service skills.

  • Ability to work independently while collaborating with a team.

  • Basic computer skills (EMR experience a plus).
