Job description

Job Summary

The Per Diem LPN is responsible to provide direct care to individuals living in a residential setting. He or she will follow the outlined plan of care, identify any health-related needs, monitor medical status and report changes to the on-call Registered Nurse, and work with residential staff to support the individuals in their community residence.
Schedule: All Shifts Available including 9am-3pm, 3am-11pm, 11pm-7am. Choose one or more

Location: Danvers, Middleton, Burlington, Topsfield

Pay: The approximate pay range for this role $42 - $43 hourly.

Please note that the pay range provided is a good faith estimate for the position at the time of posting. Actual compensation will be dependent on multiple factors, including but not limited to, the individual's experience, education, and internal equity.

Sign on bonus: Up to $3,000 paid out within the first 12 months of employment; $1,000 after 200 hours worked, $1,000 after 400 hours worked, $1,000 after 800 hours worked

Primary Responsibilities

  • Understand the clinical and social needs of the individuals’ receiving services.
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care.
  • Assess individual throughout the shift to determine nursing needs. Report to and seek direction from the RN Case Manager or on-call RN as needed.
  • Present information regarding any individual in a professional manner in all settings including meetings, phone contact, daily written documentation and /or email correspondences.
  • Responsible to assist at physician's appointments, community trips, and other activities as assigned.
  • Prepare and teach health related in service to staff and provide ongoing supervision of ADL and other health related routines of each individual.

Requirements:

  • Valid LPN license in Massachusetts.
  • Experience with intellectual/developmentally disabled individuals a plus.
  • Must have a valid Driver’s license and proof of a good driving record.
