Objective
The Patient Care Coordinator improves the patient experience at PT Plus clinics by making care accessible, welcoming, and seamless. As the first point of contact, you'll help consumers become patients, ensure proper case setup, and provide the support that allows our clinical team to focus on what they do best -Touching Lives.

Essential Duties & Responsibilities

Medical Receptionist

  • Welcome patients warmly, respond to questions, and support them through their care journey.
  • Share knowledge of PT Plus services and programs, helping patients understand their options.
  • Manage check-in, check-out, and payment processes with professionalism and accuracy.
  • Ensure all patient paperwork is completed, scanned into the EMR the same day, and securely handled.
  • Maintain a clean, organized, and professional clinic environment.
  • Support clinic operations by monitoring inventory and restocking supplies.
  • Help maintain strong appointment schedules through efficient scheduling practices.

Patient Care Coordination

  • Manage central phones, messages, and faxes with timely follow-up (within 24 hours/1 business day).
  • Reach out promptly to new referrals to welcome them to PT Plus and start their care journey.
  • Review EMR task lists daily to ensure accurate and timely patient case management.
  • Verify insurance benefits and educate patients clearly about their coverage and fees.
  • Complete insurance authorization processes as needed, ensuring uninterrupted care.
  • Communicate proactively with patients and clinical staff about insurance updates, benefit limits, and potential barriers to care.
