Patient Relationship Coordinator

We are seeking a personable and persuasive Patient Relationship Coordinator to join our team. In this critical role, you will be a trusted guide for our patients, helping them understand the value of their care plans and supporting them in following through with the treatments prescribed by our providers. You will play a key role in fostering positive relationships, addressing concerns, and ensuring patients stay on track with their care journey.

Responsibilities
  • Partner with Front Office Coordinators (FOCs) and clinical teams to facilitate smooth patient transitions from check-in to treatment and beyond, ensuring patients are guided efficiently through their care process.
  • Provide clear and compassionate explanations of treatment plans, emphasizing the importance of adherence and addressing patient questions or concerns.
  • Coordinate procedure scheduling, follow-up appointments, and other services to streamline the care process.
  • Educate patients about financial obligations, including insurance coverage, copays, benefits, and available payment options.
  • Personalize care plans to fit patient schedules and financial situations, promoting the best possible treatment outcomes.
  • Monitor patient progress, proactively follow up on incomplete care plans, and address any barriers to treatment adherence.
  • Maintain accurate and up-to-date patient records, including treatment plans, insurance details, and payment information, in the EMR system (eClinicalWorks).
  • Act as a liaison between patients, providers, and support teams, ensuring effective communication and resolution of concerns.

  • Ability to simplify complex information and explain it clearly to patients.
  • Proficient in using Windows applications and learning new software systems.

Physical Requirements

  • Ability to sit for extended periods with occasional standing or walking.
  • Assist patients as needed with walking, sitting, reading, or writing.
  • Occasionally lift, carry, push, or pull up to 30 pounds.
  • Maintain clear vision for close and distance tasks, depth perception, and focus adjustments.
  • Work in an environment with a moderate noise level.
