Job Description

The Patient Care Operations Coordinator (PCOC) is responsible for the management of home health patient schedules in collaboration with clinicians and/or home health aides and managers. The PCOC assures timely scheduling and communication with patients and clinicians/aides, ensuring patient and interprofessional team satisfaction is achieved.

Management Of Home Health Patient Schedules In Collaboration With Clinicians, Aides And Managers:
  • Ensures completed schedules a day, week or month in advance.
  • Continuous maintenance of schedule throughout the course of the day.
  • Communicate schedule changes to clinicians, aides and patients.
  • Ensure all clinicians and aides are scheduled adequately to achieve budgeted productivity.
  • Assures assignment of patients/services meets timely access to care requirements.
  • Coordinate and perform daily/weekly reports and reconciliation activities to ensure patient safety is maintained.
  • Maintain accurate database of employee profile information, availabilities, schedules, and patient demographics.
  • Process payroll reports and data entry.
  • Communicate/escalate any conflict in schedules to management.
Qualification:

Scheduling and Home Health Care experience preferred High School diploma (or GED equivalent) a must, Associate degree preferred
