Job description

Jefferson Center is looking for a non-judgmental, compassionate, and patient Case Manager to work with the Prescriber Team. The primary goal of this position is to improve client outcomes by reducing no shows and late cancellations of prescriber appointments. This includes following up with clients who have no shown or canceled, and politely and nonjudgmentally addressing barriers to attending appointments. If a patient is contacted and reports significant symptoms, the Medical Case Manager will refer the patient to the correct supports, which may include the Jefferson Center Crisis Center, the client’s nurse or prescriber, and/or the client’s therapist. The Medical Case Manager is also responsible for sending additional reminders to clients who have rescheduled their appointment after no-showing. The Medical Case Manager may also help the Prescriber Team with other tasks.

Education, Knowledge, Skills & Experience Required:

  • Bachelor’s degree in human services or related field
  • At least 2 years relevant work experience. 
  • Bilingual Spanish/English a plus

Essential Duties:

  • Maintains a positive focus and excellent customer service skills towards clients, Jefferson Center staff, and the community.
  • Works from a strength based, recovery-based perspective and takes cultural considerations into account when appropriate.
  • Effectively and calmly responds to client needs and problems
  • Timely response to phone calls, emails, texts, and other requests.
  • Immediately reports high risk/acute cases to a Medical Director, Chief Medical Officer, or the Crisis Center.
  • Multi-tasks, prioritizes concerns, and effectively manages time independently.
  • Documents all contacts in the Electronic Health Record (HER) according to Jefferson Center standards while meeting productivity standards as identified by the Chief Medical Officer.
  • Collaborates with various departments to ensure a cohesive work environment
  • Attend mandatory meetings and trainings as assigned
  • Participates in supervision by coming prepared with an agenda. Utilizes a problem-solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor.
  • Maintains appropriate boundaries with clients and staff.
  • Efficiently navigated the EHR, Microsoft Outlook, Microsoft Teams, and other technologies as indicated.

Other Duties: 

  • Coordinates with navigation, case manager, therapist and/or other resource specialists.
  • Outreaches clients to discuss social determinants of health.

Salary Grade 50- $55,000 ($26.45/hr)*

Additional Salary Information*:

  • The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
