Job Overview

Shriners Children's New England is seeking a part time Social Worker for our team!  Schedule is 16 hours per week.  Mondays and Fridays 8am to 4:30pm.

The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.

The pay range for this position is $32.79/hourly to $49.18/hourly.  Compensation is determined based on years of experience and departmental equity. 

Responsibilities
  • Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
  • Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.  
  • Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
  • Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.
  • Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
  • Other duties as assigned.
Qualifications

Education

Bachelors in Social Work required; Masters preferred

Required experience

  • 3 years experience preferred
  • Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers, required.
  • Experience in a Healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/family adherence, in a Pediatric setting is preferred

Certification

Social Work license in state of practice is required

Current Case Management Certification (CMC, ACM or ANCC) is preferred

Additional knowledge and skills:

  • Bilingual in English/Spanish preferred
  • Demonstrated competency in Microsoft Office Suite required
