Job description

At Maple Woods Manor, we are a family serving families with the highest level of care and compassion.  Our team of caregivers work together to provide our patients and residents with the ability to reach their highest level of physical and social well-being.  For all of our services, we strive to make our facility feel like a home away from home.

 We are in search of Part Time Social Worker to meet the social and/or emotional needs that affect the residentsa ability to achieve their highest level of function economically, socially and emotionally. 

 Essential functions of the position include, but are not limited to:

  • Evaluate residentas social, emotional, financial and environmental factors related to their illness, response to treatment, and adjustment to care
  • Develop a treatment plan based on assessment and input from the resident, family and other disciplines involved
  • Collaborate with physician and other team members regarding significant social and emotional factors that affect plan of care.  Advocate and negotiate with healthcare team and community agencies for resident/family to receive needed care.
  • Provide interventions to residents and family including counseling, education, crisis management, resident-centered decision making, advance directives, care planning, resource linkage, etc.

Qualified candidates will have:

  • Must be certified
  • A Degree in Social Work is preferred
  • Proficiency in Microsoft Word/Excel
  • Professional interpersonal skills
