Firefly Home Health and First Call Care Management are seeking a compassionate, dependable, and experienced Registered Nurse (RN) to join our teams serving clients throughout Lake and Porter Counties.

This unique dual-employment opportunity allows an RN to provide both in-home healthcare services and geriatric care management, offering a dynamic and rewarding career path with flexible scheduling.

The selected candidate will have the opportunity to:

  • Conduct in-home healthcare visits with Firefly Home Health

  • Coordinate and manage care for older adults and their families with First Call Care Management

This position will begin on a part-time or PRN basis, with opportunities for additional hours as our client base continues to grow.

Please Note: This opportunity is open exclusively to Registered Nurses (RNs). Licensed Practical Nurse (LPN) applicants will not be considered.

Key Responsibilities

Firefly Home Health

  • Conduct in-home healthcare visits throughout Lake and Porter Counties

  • Complete nursing assessments and evaluate clients' healthcare needs

  • Monitor and document changes in clients' conditions

  • Provide nursing services in accordance with individualized plans of care

  • Communicate with clients, families, physicians, caregivers, and care team members

  • Educate clients and families on treatments, medications, safety, and ongoing care needs

  • Complete accurate and timely clinical documentation

  • Follow all applicable home health regulations, policies, and professional nursing standards

First Call Care Management

  • Conduct comprehensive assessments of clients' health, functional abilities, psychosocial needs, and living environments

  • Develop, implement, and monitor individualized care plans

  • Coordinate healthcare services, home care services, and community resources

  • Serve as a liaison between clients, families, healthcare providers, and other care team members

  • Advocate for clients' safety, independence, preferences, and overall well-being

  • Support families navigating healthcare systems and care decisions

  • Monitor client progress and adjust care plans as needed

  • Maintain accurate and up-to-date client records

  • Ensure compliance with all applicable laws, regulations, ethical standards, and company policies

Qualifications
  • Active and unrestricted Indiana Registered Nurse (RN) license

  • Registered Nurse applicants only (LPN applicants will not be considered)

  • Minimum of two years of experience in home health, geriatric care, case management, or a related healthcare setting

  • Geriatric Care Manager or Aging Life Care Manager certification preferred

  • Strong understanding of healthcare systems and resources available to older adults

  • Excellent assessment, communication, documentation, and care-planning skills

  • Strong organizational skills with the ability to manage multiple responsibilities

  • Ability to work independently while collaborating effectively with families and healthcare professionals

  • Valid driver's license and reliable transportation

  • Ability to travel throughout Lake and Porter Counties

  • Compassionate, dependable, patient, and professional demeanor
