Assignment Length: 13 Weeks
Schedule: Monday – Friday (No On-Call)


Job Overview

We are seeking an experienced Home Health Registered Nurse (RN) for a 13-week assignment in the Fairfax, VA area. This position offers a consistent Monday–Friday schedule with no on-call requirements, making it ideal for nurses seeking work-life balance while delivering high-quality patient care in a home setting.


Responsibilities
  • Provide skilled nursing care to patients in their homes

  • Perform patient assessments and develop individualized care plans

  • Complete accurate and timely documentation using HCHB (Homecare Homebase)

  • Conduct OASIS assessments and ensure compliance with regulatory standards

  • Educate patients and families on disease management and care plans

  • Collaborate with physicians, therapists, and interdisciplinary team members

  • Monitor patient progress and adjust care plans as needed



Requirements
  • Active Registered Nurse (RN) license in IN

  • Minimum 1 year of home health experience within the last 3 years

  • HCHB (Homecare Homebase) experience required

  • OASIS experience required

  • Current BLS certification (AHA preferred)

  • Valid driver’s license and reliable transportation
