Registered Nurse

The Registered Nurse (RN) is responsible for providing comprehensive patient-centered care through clinical assessments, care coordination, patient education, and ongoing monitoring of patient conditions. This role works collaboratively with interdisciplinary healthcare teams and caregivers to ensure high-quality care and positive patient outcomes in the home health setting.

Responsibilities
  • Perform comprehensive evaluations of patients' physical, emotional, and overall health status
  • Develop, implement, and update individualized care plans based on patient needs and physician recommendations
  • Educate patients and caregivers on chronic disease management, medications, side effects, and self-care techniques
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals
  • Monitor patient progress and communicate changes in condition to the appropriate care team members
  • Maintain accurate and timely EMR documentation including assessments, interventions, and patient progress notes
  • Supervise Structured Family Caregivers, Home Care Aides, and other caregiving staff to ensure quality care delivery
  • Ensure compliance with care plans, agency policies, and healthcare regulations
Qualifications
  • Active Registered Nurse (RN) license in the applicable state
  • Previous home health or case management experience preferred
