Job description

Ohana is a leading social service agency in Arizona, providing innovative, evidence-based, trauma-informed care to help individuals with special needs reach their full potential. Our residential and supportive services prioritize safety, dignity, and love, to enhance independence for each person.

We are hiring both part-time and full-time Direct Support Professionals for our Phoenix Homes. This position requires employees to be approved drivers and safely transport members as part of their regular job duties. DSP’s play a vital role in supporting daily routines, providing personal care, managing behaviors, and building meaningful relationships with the members we serve.

Required Job Duties:

  • Provide hands-on personal care to members including:

    • Bathing and showering

    • Assistance with dressing, grooming, hygiene, and toileting

    • Physical touch/physical prompts as needed for safety and daily routines

  • Support male and female members; all DSP’s must be able to provide personal care to any assigned member, regardless of gender.

  • Document daily progress notes and required shift information

  • Plan and prepare meals; complete grocery shopping as assigned

  • Transport members to outings and appointments using the most efficient route including freeway driving

  • Build supportive and therapeutic relationships with members

  • Handle household tasks such as cleaning, laundry, and maintaining a safe, clean environment

  • Administer medications as trained and authorized

  • Assist with admissions and discharges

  • Redirect and manage behaviors using Prevention & Support techniques

  • Support members in achieving daily routines and goals

  • Follow house schedules, routines, and DDD licensing standards

Additional Duties:

  • Attend team meetings and required trainings

  • Maintain compliance with all required certifications

  • Perform physical tasks including lifting, pushing, pulling, and assisting members physically as needed

  • Uphold Ohana’s values at work and on social media

  • Perform other duties as assigned


Requirements:

  • 21+ required to drive company vehicles

  • Ability and willingness to provide personal care, including bathing and hygiene support, for both male and female members as part of job expectations

  • Flexible availability

  • Ability to work weekend shifts (Saturday and/or Sunday)

  • Reliable transportation and communication

  • Ability to pass a Level 1 Fingerprint Clearance background check

  • Proficiency in reading, writing and speaking English

  • Basic computer and documentation skills

  • Familiar with Microsoft Word, Google Calendar, and Outlook

  • Complete required training: Prevention & Support, CPR, First Aid, Article 9

  • Work full-time that is 30 plus hours including weekends and weekdays.

  • Successfully pass skills assessment and medication administration assessment
