GENERAL SUMMARY:
The Social Worker provides services to patient, family members and/or significant others to help deal with adjustment to illness problems, understanding and accepting diagnoses, prognoses or treatment of new or recurrent disease.
The Social Worker will work to implement a safe and appropriate transition to the next level of care.
REQUIRED EDUCATION, EXPERIENCE AND LICENSURE
Education
MSW
Masters Degree in Social Work from an accredited school of social work
Licensure
State of Michigan license, LMSW, or license eligible, LLMSW
Training and Experience
2 years as an MSW Social Worker in an acute care setting
Job Knowledge
The PC Social Worker provides services to patient, family members and/or significant others to help deal with adjustment to illness problems, understanding and accepting diagnoses, prognoses or treatment of new or recurrent disease. The Social Worker works closely with the Palliative Care team, which includes the NP , Physician, and Chaplain. The PC team will follow up on all palliative care consults at the hospital in a timely manner.
PERFORMANCE DUTIES
  • Assess patient/family social and emotional factors in order to estimate their potential to cope with terminal illness and death
  • Provide psycho social counseling and support to the patient/family experiencing emotional, social, and economic conflict
  • Assist patient/family in utilizing community resources and community PC programs.
  • Participate in interdisciplinary team meeting to establish, review, and revise the individualized plan of care for each patient, including exchange of information, review of problems, and sharing of professional support. Discuss patient centered goals of care, disease prognosis and advanced care planning.
  • Support the patient's unique spiritual and cultural beliefs.
  • Participate in developing and implementing appropriate measures of patient care quality and satisfaction.
  • Ad hoc activities to support educational and outreach goals of the palliative care service.
  • Provide critical psycho-social support to Palliative Care patients through the palliative care services consult team.
  • Supports the inpatient case manager/social worker by developing a plan of care for patients that are being seen by the Palliative Care service.
  • Completes referrals for patients that are going into GIP hospice and are under the Palliative Care service. This will be in coordination with the inpatient social workers and CM.

  • Uses and develops the social work process of assessment, planning, implementation and evaluation.


  • Collects and assesses data related to individual health care needs including: past medical history, current health status and treatment, necessity/appropriateness for services and setting and follow-up needed to meet needs.

  • Reassess on a consistent basis and document in the EMR including whenever there is a change in the patient's conditions or goals, when there is a change in the patient's or families preferences, and as defined by the palliative care program.


  • Provides counseling to enable patient/family members and/or significant others to utilize resources and strength to make necessary adjustments due to illness, its impact and changes to be anticipated. Include patient and family and/or support person in decision making


  • Is an active participant in multidisciplinary PC team rounds and communicates with the Case Management team/physicians information that guides the plan of care.

  • Assist in referrals for bereavement resources family and/or support person prior to patient's death.

  • Identifies the need for and convenes family meetings to provide clarity to the patient's status that result in informed decision making.

  • Is familiar with the legal requirements regarding reporting alleged child and/or adult neglect or abuse. Assesses situations to determine if report needs to be made to either Children's Protective Services or Adult Protective Services. Coordinates with the physician, nurse manager and case manager to implement a safe plan of care for child if there are concerns regarding abuse/neglect. Coordinates with the domestic violence service agency to plan counseling service and/or a shelter placement, if necessary.


  • Documents in patient's electronic data base ongoing communication with all individuals responsible for the continuum of care or regulatory issues.


  • Demonstrates the ability to use the various computer programs required.


  • Conducts support/education groups to assist in adjustment process. As appropriate, may have the option to facilitate either a time limited or ongoing group process to help family members cope with illness, as time permits.


  • Handles community calls and visits if needed.


  • May rotate on the weekends and holidays as needed in the department
