About Us: Dawson and Dawson Home Care, LLC is a trusted concierge in-home care agency serving the Beaufort County community. We are committed to delivering compassionate, high-quality care that allows our clients to thrive in the comfort of their own homes. We're growing - and we're looking for a skilled, relationship-driven LPN to join our clinical team.
Position Overview The Licensed Practical Nurse will serve as a key clinical and operational partner, responsible for client assessments, individualized care plan development, and ongoing oversight of client care. This role requires both clinical expertise and strong communication skills to build lasting relationships with clients, families, and our care team.
Key Responsibilities
  • Conduct initial nursing assessments for new clients prior to the start of care
  • Complete 30-, 60-, and 90-day reassessments to monitor client status and care effectiveness
  • Develop, implement, and review individualized care plans in accordance with company policy, state regulations, and physician orders
  • Update and revise care plans as client conditions change
  • Assist with scheduling coordination to ensure appropriate caregiver-to-client alignment
  • Participate in client meet-and-greets to establish rapport and set care expectations
  • Maintain accurate and timely clinical documentation within the agency's care management platform
  • Serve as a liaison between clients, families, physicians, and the care team
  • Support agency compliance with South Carolina DHEC home care regulations

Qualifications
  • Active South Carolina LPN license in good standing (required)
  • Minimum 1-2 years of nursing experience; home health or home care experience preferred
  • Strong clinical assessment and care planning skills
  • Tech-savvy - comfortable with electronic health records, care management software, and digital communication tools
  • Exceptional interpersonal and relationship management skills
  • Highly organized with strong attention to detail
  • Reliable transportation and valid SC driver's license
