NOW HIRING:
MDS Coordinator - Part Time

Essential Functions:

1.ย ย ย ย ย  Keep abreast of current federal and state regulations, as well as professional standards.

2.ย ย ย ย ย  Assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.

3.ย ย ย ย ย  Coordinate, manage and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.

4.ย ย ย ย ย  Assist nursing management with the coordination, management, and review of nurseโ€™s notes to determine if the care plan is being followed.

5.ย ย ย ย ย  Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.

6.ย ย ย ย ย  Assure MDS and support documentation are accurate representation of the Resident and meet regulatory and auditor requirements.

7.ย ย ย ย ย  Perform regular audits of documentation to assure accuracy.

8.ย ย ย ย ย  Assist nursing management with the discharge process.

9.ย ย ย ย ย  Perform administrative requirements, such as completing necessary forms and reports.

10.ย  Assure that established infection control and standard precaution practices are maintained when providing care.ย  Follow established safety precautions when preforming tasks and using equipment and supplies.

11.ย  Maintains strict confidentiality regarding sensitive health information of Residents.

12.ย  Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons.

13.ย  Maintains the comfort, privacy and dignity of Residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment.

14.ย  Answer and respond to call lights promptly and courteously when working in Resident care areas.

15.ย  Communicates and interacts effectively and tactfully with Residents, visitors, families, peers and supervisors.

16.ย  Attend and participate in departmental meetings and in-services as directed.

Required Education and Experience:

  • Current State License as a Nurse

  • C.P.R. Certified

  • Previous experience as a MDS nurse

Preferred Education and Experience:

One year experience as a Nurse in a long-term care setting.

Additional Eligibility Qualifications:

Knowledge and training in all aspects of MDS process.
