Job description

Wilmington Nursing & Rehabilitation Center in Wilmington, DE is seeking a qualified individual to join our growing team as a part time RN MDS Coordinator.

As an RN MDS Coordinator your primary responsibility will be to ensure that every patient has an accurate minimum data set (MDS) assessment and work closely with the interdisciplinary team to create comprehensive plans of care after reviewing the patient’s medical record and communicating with direct care staff, the patient’s physician, and family. You will play a pivotal role in helping us meet our commitment to providing Care Beyond Compare.

What you'll do

  • Complete and submit accurate minimum data set (MDS) assessments on patients.

  • Oversee care plan process including coordination of patient care plan conferences.

  • Ensure Assessment Reference Date (ARD) stays within Medicare & Medicaid guidelines.

  • Participate in monthly Quality Measure meetings.

  • Work with nursing leadership team to identify and resolve opportunities in documentation.

  • Assist interdisciplinary team in making appropriate Medicare coverage decisions.

What you need

  • Active Licensure as a Registered Nurse (RN).

  • Skilled Nursing / Long-Term Care MDS experience preferred (at least 2 years of experience).

  • Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.)

  • Excellent communication and interpersonal skills.

  • Highly organized and detail oriented.

  • Ability to work independently or as part of a team.
