Job description

Care Team Role - Intake Specialist
Supervisor:Administrator
Job Purpose:
The Intake Specialist is responsible for managing the initial intake process for referrals to Revitalize Home Healthcare, ensuring accurate collection of patient referral information, timely communication, and proper routing of clinical matters to appropriate licensed personnel. The Intake Specialist serves as the first point of contact for new referrals and supports compliance with agency policies, licensure, and accreditation standards.
Qualifications:
  • High school diploma or equivalent required
  • Experience in healthcare intake, scheduling, or administrative support preferred
  • Knowledge of home health services and referral workflows preferred
  • Strong organizational, communication, and documentation skills
  • Proficiency in data entry accuracy and document verification
  • Proficiency with electronic health record (EHR) systems and office software
  • Ability to maintain confidentiality and comply with HIPAA requirements
  • Ability to verify insurance and navigate eligibility platforms
Compensation:
Part-Time $15 - $18/Hourly Based on Experience
Responsibilities:
  • Receive and log referrals from physicians, hospitals, facilities, and community partners
  • Greet new clients or patients and guide them through intake procedures
  • Collect and verify referral information (demographics, payer source, service request orders)
  • Answer questions regarding the intake process or required documentation
  • Adhere to agency policies regarding privacy, confidentiality, and professional conduct
  • Ensure required referral documentation is obtained and forwarded to appropriate clinical leadership
  • Verify benefits eligibility and obtain pre-authorizations if needed
  • Escalate clinical questions, eligibility determinations, or care-related concerns to licensed personnel
  • Input data into EMR/EHR or client management systems accurately
  • Ensure all forms are completed, signed, and filed in accordance with policy
  • Coordinate intake workflow with Administrator, Director of Nursing, or designated RN as applicable
  • Maintain organized intake records in accordance with agency policy
  • Monitor and follow up on incomplete or missing documentation in accordance with agency policy.
